Download and complete the Library Registration Application Form.

Open the Google Form - Open Registration Google Form

Sign in using your personal email address and open the Google Form.

Complete the Google Form

Complete all required fields and upload the following documents:

01. Completed Library Registration Application Form
PDF format
02. Professional Photograph
JPEG format
03. Course Payment Receipt
PDF format — Please ensure that the Library Membership Fee is included in the receipt.
04. SLMC ID Card
Front and back of the card combined into a single PDF.
05. PGIM ID Card
Front and back of the card combined into a single PDF.

If you have not yet received your PGIM ID card, please submit a copy to the Library once it is issued.
